The CHS/SSC concept

The vision: one health identity, information that follows the person and continuity of care across the health system.

The idea

Every person should be recognised as the same person wherever they receive care, and the information needed for safe care should follow them with appropriate permission.

The purpose

CHS/SSC aims to replace fragmented encounters with continuity: less repetition, safer decisions and a health system that can coordinate around the person.

What the concept means in practice

  • One health identity that refers to the same person across participating facilities.
  • A longitudinal health record that continues across encounters and places of care.
  • Person-centred care in which authorised professionals can use what is already known.
  • A connected health system in which referrals and care transitions keep their context.

What the concept is not

  • The CHS/SSC concept is not the software. It defines the health-system outcome the software is designed to support.
  • The CHS platform is the digital implementation of the concept, not another name for the concept itself.
  • CSU is a coverage programme that can interact with CHS. It is not another name for CHS or for the platform.
  • CHS is an independently built Obosu prototype. It illustrates a government-scale system but is not currently an official government service.
